Insurance Billing

As a courtesy to you, Family First Physicians will file for benefits on your behalf with your insurance carrier. If you wish us to file for you, it will be necessary for you to supply us with complete and accurate information.

All charges for medical treatment are the full responsibility of the patient and are due in full 45 days from the date of service. This will allow ample time for reimbursement from your insurance carrier.

For your convenience, we are pleased to accept Discover, VISA and MasterCard.

Because of continuing changes in health care regulations and the increased cost in providing medical care, we have implemented this policy in order to continue to provide you with the very best in medical treatment and care.

If your insurance has changed please update our records by filling out our insurance change form, or by letting us know on your next visit.
